We are very pleased to be working with 4YP on their three-year youth social prescribing pilot project funded by Babergh and Mid Suffolk District Councils.  4YP have embedded a Youth Link Worker in Holbrook Academy. As part of a broad remit, they will be exploring what more can be done to support the transition of children from primary school to secondary school. As part of a wider exploration of partnership working between 4YP and ASSET Education, Shotley Community Primary School and Stutton CofE Primary School have been selected as the focus primary schools. 

The pilot began in September 2023, and is currently scoping how the project will achieve some key holistic outcomes that include improved well-being, increased engagement in a range of activities and positive experiences and increased resilience and self-esteem.

As a primary specialist trust we are committed to supportive positive transitions into secondary school and proud to be working with 4YP, and will look to share findings across our family of schools. This work builds on our partnership with ImpactEd, with whom we have monitored and explored our children’s well-being for 4 years.
